LO likes his paci but can't keep it in his mouth! He's 21 weeks/16 weeks adjusted. At what point did your LO learn how to replace their own paci??

About 5-6 months! It was attached to a wubbanub which helped a lot

I also gave her a paci plushie at first (which is like a wubbanub but you can connect to any paci, she only likes avent). If it was just on her chest she could sort of scoop it into her mouth around 5ish months. Around 6-7 months she could search out a regular paci and get it into her mouth from anywhere in the crib (note... the fact that she CAN does not mean she is always willing to lol. She's 10mo and she can spot a paci on an end table and crawl over to retrieve it, but if she's in a mood she'll just lay in her crib and cry for me to come get the paci right next to her head into her mouth....)
