My BFF gave us the Baby DeeDee Sleep nest, and LO loves it. Keeps him cozy all night while I don't have to worry about a blanket being over his face. We just ordered the lite version for the summer:

Newborn photography gift certificate. But that's expensive.
I like to do theme gifts, like a bedtime basket. You could put a bedtime book, pj's, a lovey, night light, etc. Or bath time with soap/shampoo, bath toys, hooded towel. Or since summer is coming up you can get a bathing suit, towel, sunscreen, sun hat.

@artbee: I like stuff like that, too. A basket that can be used for something else and filled with themed stuff. Bedtime is good, you could fill it with books, I know at Kohls there are the books that come with a stuffed toy of the main character, or maybe a breastfeeding basket with nursing pads, extra bottles, burp cloths, etc.

My favorite and most used item is my LO's two loveys. I never see loveys on a registry and my favorite brand is Angel Dear. I just gave one to a friend who just had a baby and she loves it.

I have found them at smaller boutique shops rather than the bigger chain stores (BRU, BBB)

My college girlfriends and I have always bought a gift for mom instead of baby (we all tend to give baby gifts once baby is here). They chipped in and bought me a large gift card to a spa/salon and it was so fantastic! I was able to get 4 pedicures (about every 2 weeks) and a prenatal massage, all spread out during the third trimester. I was such a happy lady!
We mostly stick to gift cards (spa, restaurants, etc) or jewelry (Pandora charms for one, Tiffany necklace for another).

I like to make baby quilts for friends and family. I also like to give a basket filled with items you might need but aren't on the registry, like baby detergent, diapers, burp cloths, and at least one book.

I want to chime in that we received gift cards to restaurants that had curbside pickup and they were AWESOME in the first few weeks of having the baby at home. Didn't have too much of a meal train with friends and being able to call in a dinner order and have DH pick it up was perfect.

To be perfectly honest? Cash. I loved the people who gave me cash, because I was saving up for newborn photography and it helped a lot.

gift cards... that we used to buy stuff off our registry because no one at our baby showers really bought us gifts off of our registry. Haha. I've never been to a baby shower where people actually used the registry, let alone where the registry was almost complete.
Favorite other thing off-registry... books. You can never have too many, and if there are duplicates (unlikely), then the extra goes in the diaper bag/to grandmas/in the car/elsewhere, so duplicates aren't a problem.
