I'm sure you're doing all this already, but this is what we try to do with bad diaper rashes:
- Change diaper often
- Allow butt to dry (or pat dry) before putting on new diaper
- Use LOTS of diaper cream (we use Desitin)

We slather on the diaper cream and try to let her air out as much as possible. Diaper-free time is good too, but we have carpet and I don't like running around making sure she isn't peeing on everything.
ETA: We use butt paste.

Wipes always make my lo diaper rash worse. So I ditch the wipes and use water and a dry wipe, or paper towel, wash cloth, anything really.
Lately I've been getting fancier and mixing up water, coconut oil and some calming essential oils for the skin and use dry wipes and it clears it right up.

Both of my kids have very sensitive skin and get bad diaper rashes quick. My go to has always been Triple Paste but it didn't help our last one. Someone recommended calmoseptine and it is the best ever. Found it at Walgreens with the incontinence products but also bought some on Amazon. We also did baking soda bath, air time when possible and switched to paper towels for wipes until it got better.

Baking soda bath after every poop or at least 2x/day. Lots of diaper free time. Aquaphor and or a zinc cream. Antibiotic ointment on any raw spots (we have an rx for mupuricin).
ETA: no wipes, just cloth wipe in warm water.

Definitely using just water and a soft paper towel or cloth instead of wipes until it clears up. That's my biggest thing.
Before she was mobile we would bring the whole changing pad into the living room and let her chill bottomless on that for a while. But I think your LO is older and mobile so not such a good option.

LO had the worst diaper rash a few months ago. It looked like her skin was burned, and it was so raw! It was like that for a couple weeks. We did no wipes, just water, diaper free time in the pack n play (she hated the containment), TONS of diaper cream + calmoseptine. It took about a week to get better.

All of the above plus I squirt a little breastmilk on her butt after wiping and before butt paste.
