I like to give surprises too. I make a note of things he mentions he wants to make it easier though. Some people are hard to shop for. I really appreciate lists then!

@Leialou: Every time I ask, DH says there's a waterproof running jacket he wants, and I can't bring myself to buy it after I just bought him a boring tool chest for his birthday. Can't I upgrade his iphone or something?

In theory I like to give surprises but I totally asked DH exactly what he wanted this year and then bought it, lol.

I try and do surprises but this year I had to go off lists for my family. DH's family is all surprises, they don't so lists.

Mostly lists. If I have a great idea for a surprise, I'll do it, but usually I don't so I'd rather just go from a list. DH and I are super boring for Christmas this year-- we both asked for very specific things, but dammit, I want my Canon Rebel! I do have one small surprise for him.

I try and gift surprises for my hubs and he tries to do the same... but we are both so bad at keeping secrets from one another that we usually spill the beans before hand. For example- this year he bought me a signed baseball and new pearl earrings (I lost one earlier this year) and he broke down and told me last week because he was so excited.

I like to ask DH for a long list and then I pick a few things and surprise a few things!
This year I got him a Nook e-reader and I think he will be really surprised.

I pick from the wish list because I'm terrible at gift giving. I just seem to lose any sense of creative gift ideas and I don't like to give crappy gifts.
I have a SIL who will ask you for a wish list because she "can't think of anything at all" and then never buy off the wish list. WTF?

I prefer to give surprises! This year I was tired and overwhelmed so mostly went off lists.
