Our entire home was painted in flat paint - colors were fine so we didn't paint over them.
Been here 5 years and they need to be cleaned! I tried using warm wash rag (just water and dish soap) and you can totally see where I cleaned!
Do I need to
a) clean the walls top to bottom?
b) just repaint the walls
c) clean some other way?

Repaint :'(
Flat paint was used in our Master bedroom and office and it is sooooo bad. I cannot get the walls clean-looking, no matter how many times I wash them.

repaint. There is just no way to clean flat paint. I hate it. When we bought our house they'd done the whole thing in flat beige and every time I tried to clean a scuff I'd get a big shiny mark. We finally just repainted everything.

repaint in a semi gloss. We had the same and the paint comes off the walls. IT IS SO UGLY.

Yes. Hate it. We will likely repaint at some point. My favorite is the spot where my son wiped snot on the wall.
I wish I had thought to ask when we built, but it sounds like another finish would have been a costly upgrade.

We did all our painting and only used eggshell or semi, way easier up clean! Though sometimes you notice where you clean because you don't realize other parts are also dirty lol, I know that happens to me.

Flat is best on ceilings, Satin or eggshell on most walls, semigloss in bathrooms and kitchens for easy peasy wipe ability.
Did you try magic eraser?

I was also going to suggest magic eraser. I used flat paint and have erased a few places. Sometimes I can still see it *right* after I do it, but it usually isn't noticeable after a few hours (maybe it takes some time to dry?).

I think you have to repaint. 
The previous owner of our home used flat paint...then DH insisted you're supposed to use flat paint so he did, all over the home. I realized I can't clean it so I suggested trying eggshell in another room and voila.
