Definitely out of a bottle - I actually can't stand drinking water out of a glass but will do so at restaurants out of necessity. I have a nice water bottle that I always carry around with me and refill as necessary.

Bottle. I usually put lemon or Crystal Light in it though. I'm not a huge fan of plain water (I'm weird, I know).

Glass, or glass bottle. I loathe plastic.

I don't really care as long as I have a straw. I drink SO much more with a straw...

I drink out of both. The real noticeable difference for me is where I am. I drink so much water at work because I'm sitting and not doing a whole lot, although at home I rarely drink any because there is so much to get done I don't think about it.

@MrsBrewer: haha yep! I do it so I won't snack. Every time I get hungry I just fill up my glass!

Bottle with ice. We do tap water and it smells and tastes terrible so the bottle keeps me from smelling and ice helps with the taste

I pretty much use a tervis tumbler at work and home. Unless we have people over then I will use glass. Tervis are our everyday glasses, so we don't have to worry about coasters since they don't sweat.
