I have used it in the past. It works really well for a lot of people. One thing that I didn't realize was how long it will take to dry. After washing, it can take like 2 days to be ready to use again. I just had one so it didn't end up being worth it to me to use it on the regular.
As far as the routine, generally you need to lanolize it every 2 weeks. The process is to fill up a sink with warm water and dilute some lanolin in it. Let it soak and then drain the water out through the cover and hang it to dry. Washing is usually just done as-needed (AKA poop). I would hand wash my cover with special soap whenever it was time to lanolize and then do the lanolin process.
So... yes it is a separate routine and can be a bit of a hassle unless you have several to keep it going. DS wore his wool cover over a fitted for overnight, but a lot of people use them all the time or over other types of diapers for extra leak protection.
They do make these cute wool longies that would be nice for winter. You could have the fitted underneath and the wool pants on top, not need for more clothing on top.
