Is there anyone on here who has gotten custody of their LO from the hospital immediately following birth?
If so, can you give me a little rundown on how it all worked out? I know every situation and hospital is different, but I'm curious.

I haven't adopted. My sister has adopted both of her kids from the hospital. My friend also took hers home from the hospital. In her case she was even there for the delivery. The baby was released to them when it was ready.

We took our son home from the hospital. We visited with both the birth mom and our son in the early afternoon. Then they asked us to leave while they signed paperwork. In our state the birth mom can't sign until the day she is leaving the hospital. They took the baby to the nursery while the birth mom was released. Our social worker called us back and we said good bye to the birth mother. We gave her a small tinkerbell necklace. She said it was her favorite character and one of the reasons she picked us was our love for disneyland. When she left they took us to the nursery and gave us our son. We say and signed our portion of the paper work and discussed our post adoption meetings.
The funny thing is the baby had to be discharged to the social worker, not us, and its hospital policy to take the baby and mother out in a wheel chair. So our social worker was wheeled out of the hospital with our son. At the front of the hospital she handed him to us and checked our car seat. Then we drove home where all our family was waiting for us,

I have never adopted but have two close friends who have (one couple has done it twice). Their experiences are somewhat different, as you said the hospital's policy plays into it. For the couple that adopted twice: the first time the mom was given a hospital room right next to the birth mom's room so she was there the whole time the baby was in the hospital. The second time they just visited with the birth mom. My other friend was able to be in the operating room for the c-section and was the first person to hold their son.
