Ok no! We are dealing with this as well. Works for us usually quickly:

Don't wipe. Rinse them in a sink
Naked time
Canesten cream (it rubs in really well on its own) mixed with a bit of diaper rash cream

My friend told me today actually that all diaper rashes are a form of yeast infection. (She learned that in pharmacy school - and she's a pharmacist so I believe her).

I put Alba Un-Petroleum Multi-Purpose Jelly on top of the diaper cream and find it really helps protect LOs skin.

We have a nasty teething, runny noise rush right now that's just starting to clean up. LOs been an unhappy camper. I wish i could get his butt under the sink, but he's too big now.

I use aquafor. If it's candida/fungal your doctor will probably have to prescribe something though (happened to us when I was trying cloth diapers)

Try a different diaper too. Sometimes the chemical j. The diapers cause irritants.

@Littlebit7: Is it bright red/seem like it could be from the acid? My ped recommended Mylanta (applied via cotton balls) for DS1, and I've used it a ton for both boys. She recommended washing via peri bottle or cotton balls, then Mylanta, let it dry completely, then diaper. Sometimes I also use Vaseline.

I've always had very good luck with Boudreaux's Butt Paste in the green tube.

Our nicu recommended Boudreaux's Butt Paste in the red (highest zinc oxide content)

@Littlebit7: definitely try the mylanta then. It sounds like that kind of rash. Good luck!

From a mom at daycare:

our pediatrician has quite the theory about diaper rash that I thought might be helpful to share! He says that during the day, rashes should be lathered with aquaphor or something similar, which creates a moisture barrier between the skin and the diaper and also helps ease any burn they might feel from urine on the rash. Actual diaper cream should only be applied at night and it should have the active ingredient of zinc oxide, which is the drying/healing agent. He thinks that too much of that one (day and night) actually can make the rashes worse by irritating them.

* We love the Red Butt Paste at night. Very high % of Zinc Oxide.

@T.H.O.U.: I tend to agree about using too much zinc cream - I think it irritates LO's bum. We have found the Motherlove Diaper Rash & Thrush ointment helps to heal/prevent anything that may be becoming a yeast rash. It seems like a small container for the price, but you only have to use a little and it goes a long way. We apply that and often put Aquaphor on top as a barrier.

We recently had a pretty bad rash too. This worked -

NO wipes, not even cloth w/ water. Rinse her butt. Her we squirted water at her butt and gently pat.

Then let her hang out to air dry her butt. We don't want her to walk around so we put on a video and just let her lay there.

We put on straight up cornstarch i/o baby powder.

Then LAYER on whatever cream (Desitin works fine in our case), and then layer on A+D, thick thick thick. We don't put her diaper on too tight, and if she's wearing a onesie I don't button it to tighten that area.

A&D has a lot of Petrolatum which I wasn't super comfortable with and I can't stand the smell.

When he has had a bad rash, I would use the zinc for 24 hours or something and then go back to just using that overnight and coconut oil during the day.

for every diaper change we use A&D ointment (not the white) and cornstarch powder

and then do A&D and powder over it

I've used every cream and the Paladin works the best, we discovered it when my older LO had runny poops for a month and were desperate

Our ring around the anus rashes were a dairy allergy symptom. Maybe try a probiotic?

Hmm. Can you remember what exactly she ate the BRAT type diet day and what exactly she ate the next day that was more normal?

We love Triple Paste, and we have a prescription cream that also works wonders - we switch back and forth between the two during the day and it usually clears up within 48 hours.

As a side note: Do you think it could be a yeast infection (starting out as little red dots)? If so, you will need to put a layer of Lotramin cream on it (athlete's foot cream) and then cover with a thick, zinc oxide-based cream, like Triple Paste.
