Sanitary wipes -- the name escapes me, but we used them for when toys would drop on the floor or her pacifier. Munchkin makes them & they're in one of my guides!

Something I now keep in my diaper bag (for myself) are a couple granola bars so that if we're out and I'm famished I have something quick.
For the baby, maybe a pacifier pouch? Travel toiletries? Re-fillable travel wipes case?

I've been giving my new parents friends 'homemade wipes' because our hospital recommends wiping with nothing but water for one month.
I fill a wipe case with 1/2 sheets of Viva paper towels and tell them to wet them when the baby comes home. So far it's been a hit.

Those prefold diapers as burp cloths! Bonus points if you do something nice like sew middle panels on them and have them embroidered. I freaking LOVE mine. So much. The NoseFrida is also a win. And for nail clippers, I like the Piyo Piyo scissors.
I also keep a makeup bag in my purse filled with my stuff--lotion, sanitizer, lip gloss, advil, chapstick, etc....this way the baby doesn't accidentally get my stuff and if i transition between diaper bag and purse, i just pull the makeup bag out and plop it in my bag!

Everyone has said it but BOOGIE WIPES! Those were so key for my lo who hated the snot sucker.
