Honestly, E is so heavy that this is something I think about. I unload the groceries into the car from back of the car and keep a foot on bottom of the front of the cart to balance out his weight.
(Ideally I've parked right next to a cart return so I can strap him into his seat (with his door still open and the car on), then unload and step 5 feet away to return the cart).

Whoa! I never thought about that happening.
I always put Chloe on her car seat first then lock the car. I have nightmares of someone snatching my kid while I unload groceries. I have a hatch lift trunk so she gets plenty of air.

Ugh, cart accidents scare me. LO 1 fell out the other day...MIL let her sit on the handle bar and wasn't paying attention.

LO loves sitting in carts (especially in Target) but with their carts, the strap goes across his chest and kind of pins him back against the seat. I could see this happening more with lap belts, since he could lean over the bar...
That being said, I will definitely make sure I don't unload the cart while he's still in it (I just did that today, actually).

Crazy. Luckily, I always put my kids in the car first before I unload the cart. I worry about them wiggling out of the cart while I'm not looking.
