I don't know what I did with mine, but it was short, sweet and to the point. "As of March 6th, I resign my position to stay at home with my new baby. Thank you for all the opportunities that you have given me."
Or something like that.

Heres mine:
Dear Boss,
I hope you are well and it was nice chatting last week. As we discussed, please allow this letter to serve as my 2 weeks notice to (company) in advance of my resignation.
With the addition of our six week old daughter, I have decided to focus on raising our family for the near future. However, I have truly enjoyed my time with (company), and I don't want my resignation to foreclose the possibility of future opportunities with the company.
I learned a great deal working under your leadership and I hope to maintain a continued relationship with (company) and its employees.
Thank you again, and kindly let me know if there is anything further you need from me.

Do you guys have to return to work in order to keep benefits? I was told that if I don't return to work, I can be asked to pay back the companies contribution to our insurance for the duration of the maternity leave. My boss (who kind of says these things with a "wink, wink, nudge, nudge") said I can come back for just a day, and then give my notice, but I have to return.

@silva: I was told that I had to return to work, otherwise I would have to repay STD and insurance, but my boss said since I gave my two weeks notice (and the company opted to waive those two weeks) I wasn't on the hook. Often times there is a time frame afterwards that you have to work, but I think as long as you leave in good standing (and dont just "not show up") most companies aren't that strict about it.

Are you planning on quitting after taking your maternity leave? I would encourage you to be honest and quit before. I had a realllly hard time finding a job this summer because I was pregnant, and part of it is because employers worry women will just quit after maternity leave. I would say, help us working mamas out and if you know you are going to quit, go ahead and do it!
