A carrier, 100%.
I tell people all you REALLY need is diapers, boobs/bottles, and a carrier. Seriously that is my survival item.
Whether that be a mei tai, a ring sling, a stretchy wrap like the moby, or a woven wrap (my personal carrier of choice)... a baby carrier is a lifesaver.

I was going to say ergo and miracle blanket, @Little Misters Mom: is right, the excersice ball was a complete and total necessity.

1. Rock n play. LO has silent reflux. We got it when she was 4 1/2 weeks old. She slept so much better and most of her symptoms went down dramatically.
Runners up:
*carrier (we use a k'tan and a non stretchy wrap a friend made. We have an ergo for when she is older. All have been valuable for us)
*miracle blanket
*merino sleep sacks
*boppy - helps save my back when nursing. Also helps me when I want to hold her and my back is playing up.
