Your family doctor may refer you. The first thing they'll likely do is send your partner for a semen analysis. It's so easy to test whether men are having issues or not, so they always try to eliminate that first. They'll likely do cycle day 3 and cycle day 21 bloodwork to confirm ovulation. They'll also test for a few different hormones. They might also test for thyroid issues. And perhaps send you for a HSG exam. @Shizaz: You're welcome! It's so nice to have the information. I forgot to mention that I ended up getting pregnant on the second cycle of clomid, metformin, estadriol, and progesterone. I have atypical PCOS - meaning all I have are the cysts without any hormonal symptoms - never showed in any bloodwork but a doc found it in an ultrasound. And this pregnancy has been an absolute dream! I am profoundly lucky and very aware of it.

Based on what you said is why I might suggest charting.

You can get a positive OPK because your body is trying to ovulate, even if ovulation doesn't occur. But your temp won't shift unless your hormone levels change because it has occurred, so charting might help confirm if you are or are not ovulating.

And if you aren't, a skilled doctor will be able to use your chart to try to pinpoint problems. Your temp should be rising and falling at specific points because of your hormones changing, so if they aren't, they can better see which hormones to test first.

@Shizaz: HB ate my earlier reply. I did not think I was ovulating with regularity. What @marym said was true for me, my body would gear up and then not ovulate. I did temp for a few months which never confirmed ovulation.

ETA: This is what made me insist on testing by my doctors before we'd even been trying a year. I know I had basically zero shot of getting pregnant given that I didn't think I was ovulating. Also no, I had no pain from my cysts. My cycles eventually became regular at 30-32 days. And my periods were not heavy. I NEVER would've thought I had PCOS.

@Shizaz: we ended up getting referred to a specialist in December for a full work up. Because we had been getting pregnant ok, but have had multiple losses, they did tests on me first. I had to do blood tests on CD3 and 7DPO and a hysteroscopy and tube flush. My hormone levels were off so I was started on progesterone and baby aspirin for a blood clotting disorder. My DH was also signed up do a SA but we found out we were pregnant shortly before he had to do it. I'm thankful that we went and did all the testing and for us it seemed like it was a quick fix for us. I hope that your doctors can also figure out what's going on with you and that a pregnancy is in the near future!
