My LO is 4 months, too. If it was fussing I'd probably let her fuss for 10 minutes or so. If she didn't go down or look like she was about to go down in that time I'd probably go in. If she was hysterical I would go in right away.

At that age, I would let her fuss for a few minutes to see if she would go back to sleep. If she started crying, I would go in and rock her back to sleep.

If it was a hysterical (like painful cry), go in immediately and pick up/rock back to sleep. Usually it was just a nightmare or something and she'd all back asleep in my arms within minutes.

If it were some low cries/fussing, just watch on the monitor and hope she settled back down. If crying escalated, I always went back in. Mine's an escalator and then it's hard to get back to sleep. Usually the sooner I go in, the quicker and easier it is to get her back to sleep.
