When DH's grandmother turned 90 we got her some of her favorite flowers and planted them in her yard for her. She didn't really have space for anything inside her house.

Have her friends and family write their favorite memory about her and read them all to her at her birthday party.

My grandma is 95 and I just gave her cash for her bday. She likes to have cash on hand so she can buy other ppl gifts and food. She loves shopping for others. Like you, she has way too much stuff in her home.
Also, it's acceptable in my culture to give cash to elders so I took the easy way out

All of the elderly in my life and that I worked with have loved time. Your time with them. Perhaps take her to dinner and shopping.

For my grandma's 80th birthday we arranged dinner at a fancy restaurant downtown, took her in a limo and then went to the casino afterwards. My mom covered the limo - we all (3 granddaughters) split dinner and gave her $100 for play at the casino.
She loved having the special night with all of us!

I agree with flowers and something handmade from G. Or even a framed picture of her and G.
If possible spend time with her. I think at that age they just want to see the future generation.

My Nana always wanted a large group picture framed of all her family so she could show us off to visitors. Maybe a photo book from Snapfish or a similar site with a section for each branch of the family and their favorite memory of her?

@LovelyPlum: DH's grandma is 94 and last year we did a photo book for her of her family - she was starting to forget names.
It started with her and her DH on the 1st page
Then it had her kids and their names
Then it had each of her kids with their families and names under all the pictures
Then great grandkids

@LovelyPlum: Does she have a ton of great-grandkids? You could do a great-grandmother's necklace for her with each child's name or birthstone?

For my grandma's 90th we did a photo book for her. I find that older people are so impressed by photo books because they would never even know how to have one made and it looks so fancy to have all your photos in a hardcover book like that. My grandma loved it and always showed her friends who came over!

We did a photobook and a Pandora bracelet for my great aunt (grandmas sister and like a second grandma to us) when she turned 90, all the kids got charms for the bracelet. She loves it.
It's her 92nd this weekend and I have no idea what to get her, haha
