This is to someone far away. The new baby has an older sibling. The mom pretty much buys everything she wants/needs and has kept everything from her previous child (same gender) in preparation for this one.
My mom already bought them a Marpac. Thanks, mom. Haha.

A book with the name (we have a Felix Featherbottom book and LO loves it and it's such a keepsake!)
Otherwise, new onesies, new bibs/burpcloths, A&A blankets,. These are all things I'm replacing with my new baby (same gender)
Or if you know what the nursery is some artwork or momento to fit in there.

What about stuff for her if she has everything she needs for the baby? A package of Earth Mama Angel Baby stuff, like the nipple cream and bottom spray, tea, gift cards for takeout restaurants they like, etc.

A& A swaddles were a huge hit for last baby shower. They didn't have those yet when she had her first two kids. I also love to give Angel Dear loves because K loves her so.

If she lives in a large enough city, there are meal delivery services. You go on-line an pick out an entree and sides, and they deliver it straight to your door. Seriously, it was the best gift ever after having a new baby.

I also think something personalized or new towels, washcloths blankets. Meals are a great idea too!
