I am expecting my third and have one of each already... I would love a nice carrier or ring sling for this one! I also like the pampering for her idea... Maybe a gift card for a prenatal massage?

My SIL is pregnant with her third and fourth daughter right now and we bought her a prenatal massage gift card and she was so so happy. My MIL also gave me a coupon for babysitting to add to the gift card.

I only have two kids but this recent time around, I've really appreciated personalized stuff for my new LO. I like that he has some nice things just for him, considering 99% of his stuff is hand-me-down. For example:

As much as I loved my ring sling & swaddles as gifts, my baby did not, so they never got much use. You just never know.

I'd do sown thing for her - a gift certificate, mani-pedi, massage, restaurant. Or a nice robe.

My sil had a third and she said she felt like people sort of neglected the last baby. Not as many sweet thoughtful gifts as the first and second. She said she didn't need a lot of gear. You could get something for mom now and then wait until after the birth and get something monogrammed or personalized for baby once you know the deets.

The things that made me happy....

New hooded baby towels, my old ones were ratty from heavy use with my older two.

Monogrammed stuff. We don't"need" anything so I have a hard time buying things we already have but it's nice for her to have some things that are only hers.

People to hang out with my older two so I can sit and gaze at the baby. My oldest has me staring at her 24/7, this one gets fed and then popped into the swing or carrier because my other two need me.

I would say something for her - if she likes to be pampered. and I would do some special things for just the little one since he or she is going to get a lot of hand me downs. I agree, a cute monogrammed bath towel or something that's just his/hers.
