I was at my friend's rehearsal dinner years ago. It was at a fancy outdoor restaurant in Malibu that had lots of hanging vines/flowers.
I drank all my coke, so I picked up my glass and got some ice to suck on. Well, something other than ice ended up in my mouth (felt like a cherry stem). When I pulled it out,

OMG! EWWW! I hate bugs! Ok aside from the time that I found maggots on the bottom of my raisin box..... When I was in 5th grade, I had a glass of orange juice next to my bed on my bedstand. In the middle of the night I got really thirsty so I reached for the glass and drank it. Then I started feeling all these things crawling on my face, mouth and inside my mouth. I turned on the lights and the OJ glass was COVERED in ants!

Gross. We eat outside a lot, so I guess we should just expect bugs. But once I was at a restaurant and asked for a new drink because there was a bug in it, and the waitress said no because it will just get more bugs.

I was at a shabu shabu place in Chinatown, and a ROACH crawled out of my veggie plate! I told the server immediately and they replaced it. The best part? We still got charged for our meal.

I once found a fly in my pb&j (in my mouth. I felt something funny and spit it out). I never ate a pb&j again.

Ewww! This post gives me goosebumps! I had a hair in my food at a local resturant once and it grossed me out completely. I also had a dead fly floating in a drink that I had just taken a sip out of, I started to gag.
