We had to pay a $955 deposit that covered the IUI, sperm preparation, one ultrasound, some lab work and the first beta. Then we had to pay as we went (I needed a lot more than one ultrasound for each cycle!). Each ultrasound after that was $160, the lab work was about $75 and the trigger shot was $90. I paid $900 for each 900iu vial of follistim until I found out that the cvs special pharmacy sells it for $300!
We did two cycle and I think it ended up being almost $5,000 out of pocket.

@mrskansas: I'm part-way through my first IUI cycle right now! We are doing Clomid because I already ovulate on my own so can't help you with those costs, but we are doing ultrasound monitoring with a trigger shot. Our health insurance does not have infertility coverage either *however* there is a loophole of sorts that does cover diagnostic tests- i.e. ultrasounds, etc- even for infertility for our specialist copay cost! I'm sure your clinic knows to look for it, we would have had nooo idea ourselves.
The IUI itself will be $450, $20 copay/each ultrasound, Clomid was $20 & Ovidrel was $90 (both not covered on our insurance). I don't know what the final tally will be for the cycle of course but we were both really surprised by the cost- we were expecting $1,500 to $3,000/cycle!
Oh, and we live in Orlando since I'm sure location plays a role in costs as well

Monitoring OOP is about $275 for u/s and $225 for bloodwork at my old clinic. If you can monitor at an ultrasound facility and do blood work at a Quest or Labcorb, they may have to code as diagnostics testing and it may be covered. They are technically not a fertility treatment center, so I don't think they would use fertility treatment coding. The sperm washing was $180 and the procedure (my clinic did it kind of weird) was an "outpatient in office procedure" and was $400. And somehow I don't think that ended up being coded as fertility either, cause I ended up just paying the $40 copay for it. It's really hard to know how they might code if they try to run it through insurance first. I would see if it's at all possible that they even just try to charge insurance to see if any of it ends up being covered.

I can't speak for the injectables because I just took Femara but the IUI itself cost me $420 (for the sperm wash and the procedure) without insurance. My monitoring U/S was $170 but that was with insurance.

I wish I'd kept better notes about our expenses when doing our treatments. It wasn't even that long ago but I can't remember. I can tell you we spent approximately $10,000 out of pocket (maybe a bit more) on 5 rounds of treatments. We did three rounds of injectables with TI and two rounds of injectables with IUI. If I remember correctly there was a flat fee, maybe around $1500 for the IUI (covered ultrasounds-monitoring appts/ actual IUI) and then we had to buy the meds separately and they were about $750-$1500 depending on dosages.
Nothing was covered under insurance for us.
