For those that have one which bank is it through?
Is there an application and/or maintenance fee?

Hmmm...I'm answering your question with a question - do you get a choice at your company? I can't remember who ours is through but we have no option but to do it through them. It's not like a Wachovia or BOA, though....

@regberadaisy: what's a HSA? I've heard of FSA before but not HSA. I'm going to assume the SA part means Spending Account.
ETA: Oh, you put it in health. Oh, so it must be Healthcare Spending account. (?)

@bluestriped bee: HSA is a Health Savings Account that is usually associated with a high deductible health plan. It's different than an FSA b/c the funds can roll over from year to year.

Mine is through Wells Fargo. There's a $4 maintenance fee, but I can't remember if it's quarterly or monthly. No application fee though. I LOVED ours. I was sad when we had to switch to DH's insurance at his new job and they don't offer it.

Ours is through Momentum Bank or something like that... It used to be different but the bank got bought out. I know there is a 2$ monthly fee on it.
