Endometriosis. Before I went on the pill as a teenager, my periods were so painful I would lay on the bathroom floor wishing I could die. After 11+ years on BCP, the endo moved and my pain is now concentrated around my ovaries so I get the terrible cramping around O. It's sharper and more of a burning sensation.

PCOS, though my gyne suspected endo. He was a pain specialist & I chickened out of the laparoscopic surgery.
My PCOS was diagnosed in 2010, and my OB didn't see any signs of endo when I had my section.

@septca: I've had 3-4 times yesterday and 3-4 times today where my cramping was so severe I had to stop and bend over. It's worse than my contractions were before my water broke.

@septca: I had the same pain. I would spike a fever with every period and be in so much pain i would throw up. Bcp didn't help as a teen, but as a college student my gyne let me try different pain meds. Those helped, but my periods didn't get better until after I had Chloe.

Right before I got pregnant I was undergoing testing because my doctor suspected I had PCOS (she mentioned Endo, too but was leaning towards PCOS). I got pregnant and switched to an OB and my previous doctor told me not to worry about the PCOS anymore since I got pregnant so fast.
But I'm actually going through the process again because I've got some symptoms. Though my periods are no longer as painful as they were pre-baby. OTC medications actually work for my cramps now, no more prescription drugs!
P.S. Mine was both--it could be off and on and constant. I never knew what I was going to get month to month.

I get cramping the first day of my period bad enough that I have to sit with a heating pad all day. My RE thinks I may have endo but not severe enough for it to affect my fertility.

@swedishfish: that's how mine is as well. The pain now - at O - is so hard and sharp it has me doubling over and breathless. The pain has been much better since I had DD, but it gets worse as the endo grows (same idea with PCOS cysts, I imagine).
@spaniellove: It always seem weird to me when doctors say stuff like that. It only takes a little to cause a lot of problems. When I had my lap, my doctor only found a small amount of active endo growth. But what was there was completely blocking both tubes, which was obviously causing huge IF problems.

@septca: I wonder if he would have pursued it further if he hadn't found a more obvious cause for my IF. At the time it seemed like he was putting it on a list of possible causes.

i have very painful periods & serious discomfort around O. i also have PCOS and a very small septate uterus (septum in the uterus). they say when the septum is removed, my periods should be more comfortable. the cramping for me is worst right when AF is starting, and in the evenings.

My periods are horrendously painful, but no PCOS or endo...just awful cramps, sometimes I vomit from the pain. Ugh!

Just painful periods as far as I know. My primary care suspected endo and sent me for a meeting with her OB before we started TTC. The OB said she didn't feel I had strong enough symptoms to warrant the invasive surgery to check for endo. She wanted me to TTC for a while first. I got pregnant on the first try twice so if I do have endo it does not seem to be affecting my fertility.

endometriosis. i take vicodin, but also take bc pills back to back so i don't have a period. cuz periods suck. and so does pain. and i aint' got time for that

Mine were (they got way better after my first pregnancy, even though it was a m/c) just painful periods as far as I know. I never looked into finding out if there was an underlying cause, and we've had no fertility issues. But they were awful. I couldn't work on whatever day I started.
