People who make excessive noise when they chew (or chew with their mouths open)
Bad posture
When people hold their fork like a shovel
If I get a bit of dirt or a small stain on my clothes it eats away at me for the rest of the day.

When someone is playing music late and night and the bass hums and thumps at your house.
When someone is late/waiting for someone.

Loud breathers, when my husband drives and is inefficient (ie. won't change lanes), when people don't take their shoes off in our house when it's obvious we don't wear shoes inside.

It's interesting to see how many have the loud chewing thing! That's my husband's #1.
I hate dirty sinks. If I go somewhere and the sink is dirty I usually end up trying to secretly clean it! I've been caught a couple times and I'm always like, "I'm sorry I have a problem!"

People who say "good" instead of "well" when using the word as an adverb. Hate hate haaaate it when my in-laws say "you're doing so good!" to my kids.

Loud eaters and people that don't follow through or procrastinate. The second one especially. I get really frustrated when I expect something and then it doesn't happen for days or not at all.
