In honor of the boys birthday, we want to donate some baby items to the NICU that one of the nurses said they can always use, like onesies, blankets, mittens, etc. I know it's not correct etiquette to mention anything about gifts, lack of gifts, etc., but I'd love it if our guests knew about our project do they could donate items if they chose. Is there any way to let them know besides word of mouth? Would an insert with the invite be rude? If not, what would be the least offensive wording?

I think an insert in the invitation would be fine. Presumably you know the people attending your boys' birthday pretty well. Unless anyone you're inviting is a real stickler for etiquette I would put an insert in. As a guest it wouldn't bother me at all. Just be sure to write thank yous for the donations

What we did was to send out a facebook message to all our friends. We didn't expect our family to donate because they are big gift-givers, but gave the option to friends. Up front I told people not to bring gifts...that way the set expectation was that we would be happy not to have gifts. Then we said that if they wanted to bring something, we wanted donations to a local children's home. Then I put a list down of items less than ten dollars that they could donate (deodorant, gift cards, board games, etc.) I also said that if they wanted to offer a traditional gift, that would be great as well! That way no-one felt hemmed in.

Could you include a second invitation of sorts? Something like:
In honor of the boys turning one, we are throwing a "party" to honor the NICU that helped them when they were small. We would love for you to celebrate these people with us by donating small items they can use to help other babies, like blankets, toys, etc. Here is how you can participate: (enter details here)
In reality, it accomplishes the same thing as an insert, but you could have fun with it-let the boys do the "inviting", draw up a cute design,etc. just a thought... But I love your idea!

I would just include a note with the invite. It's no biggie IMO.

I'd say something about how they were in the NICU when they were born, and how you want to honor that and give back by donating x,y,z, and you welcome the guests to join you. That's sweet of you guys
