These are probably so basic, but curious:
If you change a diaper that's only wet, not dirty, do you still use a wipe? It's only really recently that she started not having a dirty diaper with every change!
When you put diaper cream or aquaphor on do you use you finger, or rub it into the diaper then rub the diaper on the butt?
Also- hoe do you know when to go up in sizes- leaks?

I use diaper cream and rub it with my finger. I kind of just have to guestimate to go up a size, I have a tiny baby. When he was having bad leaks and blowouts in NB, 1s, and 2s, that's when I knew to go up a size, plus the diaper wasn't falling off him.

I always wiped with water, regardless of pee or poop.

I always use a wipe - if it's just pee, I'll only use one.
I apply Aquaphor with my finger directly on her skin. As for sizing, I could just tell things were getting too snug and she would also have more blow-out leaks through the top of the diaper.

I only use a wipe if she poops. I just squeeze the cream straight into her bottom (without touching the tip on her directly, to keep it clean), and as far as sizes, I could just tell when they started to get small.

I always wipe off his bum, because sometimes he will still have a little residue back there. I always squeeze the ointment onto his bum and I don't rub it in. If he's looking a little red, then I use triple paste, which I wipe on (it's in a tub). I go by the sizes suggested by pampers, or until he starts having frequent blowouts. We've only gone up one size; from newborn to 1s.

I always use a wipe! I apply diaper cream with my finger. And I go up a size when she reaches the weight for the next size, usually there are more blowouts at this point too.

I usually use a wipe when the nappy is just wet, but if i'm in a hurry i skip it sometimes.
I rub diaper cream in with a finger
and I just follow the weight guidelines for sizes!

I pat her dry if she pees, and I spot check for goop and wipe at that point if it's necessary. I put the diaper cream on with my hand, but luckily she rarely needs it. So far for sizing, we go by how snug they are on her, and how many blowouts. Different brands are differently sized, so even though she's still in 1s, we've worked our way up from the smallest to largest ones.

Wipe at all changes, ointment with my finger but only use it occasionally, size up when the last size is feeling too snug which for us is usually before the weight limits.

I always use a wipe.
For diaper rashes, our pedi told us to apply cream like cake icing. I squeeze the cream directly into her bottom without letting the bottle touch her body, then apply with my fingertips.
I go up a size in diapers when they look like they're too small. We've never had a leak or blowout problem. We use Pampers who had a size/weight chart & its been pretty accurate for us!

I use a wipe 90% of the time. But every now and then if we're doing something like a quick change in the back of the car and its just pee then I might skip the wipe. I use my finger to apply cream. I went up a size when the were tight around her little thighs.

I put cream on by squeezing it directly onto him without letting the tube touch him and then spread it with my finger, but only apply when it's necessary.
I use the size guide on Pampers to decide when to go up in size. It's worked so far!

I always use a wipe. I think the urine would be irritating to her skin. For cream I put it on my finger and apply!
ETA: the cream definitely gets on the diaper but i think you want to avoid that as much as possible so it doesn't interfere with the absorbency of the diaper (especially with a barrier cream).

I always use a wipe. Use my finger for cream. Then either wipe excess on diaper or just a wipe to get off. We use triple paste in a jar, so have to scoop it out more.
I have gone up in sizes based on when they start getting too tight her thighs. She has chunky ones do she needs it there before around the waist.

We usually use a wipe - like @MamaMoose said, now and then for a quick change we don't use a wipe for just pee.
We don't use diaper cream (pedi and hospital nurses said to only use cream if she had a rash, we've never used it and she's never had a diaper rash).
I go up a size when the diapers seem tight/small on her. LO has chunky monkey thighs, so it's obvious when we need to move a size up to cover the upper-thigh-chub

I always use a wipe and when using diaper cream I use my finger to put it on (ours is in a small tub so no way of putting it on other then using my finger, I also put it straight on the skin, not on the diaper.

I always use a wipe; I figure since I wipe after peeing, she would probably appreciate the same.
I've only used diaper rash cream once when she had some irritation from formula. I applied it with a clean finger.

@yoursilverlining: do you not use anything on the bum? The nurse in the hospital said to always use Vaseline or aquaphor to protect her bum and make it easier to clean the poop of without rubbing. We use aquaphor unless she looks irritated, then we use the butt paste for a few days.
The nurse also squeezed the Vaseline into the diaper and then rubbed that into her butt, which is what Dh still does. But now we have a tub of it so he smooches the diaper ok it then runs that on her butt, which I think is wasteful. He thinks using a finger is gross.

I always use a wipe, but just one if it's for pee. We use coconut oil as a diaper cream, and I just wipe in on with my finger... then wash my hands (of course).

Always a wipe. Cream w/ finger, use a wipe on finger to finish dressing her. ( usually only do this after bath before bed, also in middle of the night when she was a nb) go up when there are leaks or if it looks too short especially on the back side. Too short = leaks/ blow outs!

Apply the butt cream with my fingers. Always wipe even when it is just pee. I've found you can tell when you. We'd to go up a size as the diaper is tight. We have to go up a couple pounds before what the package says is the upper weight limit. When I think it is time to go up in size I try a couple in the bigger size before finishing the smaller size lack.
As for the vasaline to make the poop wipe off easier, we did that for the first few poops as that is hard to wipe off. After that they wipe up easily.

I don't always use a wipe when it's just pee but that's because I know DH always does and he actually changes more diapers. Mom is lazy that way.
At this point we use q-tips for all ointment application. (I realize it probably won't work so well when LO is mobile). So many of the ointments are waterproof, smelly, and hard to wash off by design. I find it much easier not to touch them at all and I think I use less ointment.

We don't always use diaper cream or Vaseline but when we do I just use my finger. I don't see how putting it on the diaper first would get it on good enough.
I could tell she needed a bigger size whe she started leaking a lot. Or if it just seemed too tight.

I always use a wipe.
If I need to use diaper cream I put it on my finger and then wipe on her.
I always had to switch sizes before the weight limit - I judged it by how far up the diaper came and how tight on her thighs.

I always wipe her. I use anti bacterial hand gel on my hands then use my whole hand to rub the bum cream all over her bum and into her skin. We use Bepanthen and she hasn't had any irritation as yet.

I always wipe. I suspect that daycare does not wipe when it's just pee, and frankly, DS was coming home smelling like a little homeless man some days. Thankfully he is potty trained now.
As for cream, I use it at the first signs of rash/irritation and I apply it with my fingers.

I have boys and never wiped for pee except in the mornings since it's been there for a while. They've never had issues except if they pooped and I don't change them right away. I use a q-tip for aquaphor or diaper cream. And I go up in size if he is consistently leaking out of his night time diaper and it's super full when he wakes up. I sometimes forget to pull the tabs tighter and he'll leak out of the larger size. Haha!

@Foodnerd81: we haven't used any diaper cream at all since the first week and so far our baby hasn't had any irritation at all. Of course, it's only been 7 weeks.
The one time we tried it, she got a bit red, so we haven't used any since.

I don't always use a wipe for just pee. We rarely use cream but when we do we use our fingers to apply it. We use cloth, so I don't know too much about sizing up 
I don't think you really need to use cream or something at every change. I agree with a pp who mentioned the nurses may have meant it just for those first tar-like poops.

I always always use a wipe. I wouldnt want pee sitting on my skin and I don't want it sitting on my los skin to irritate it and make him. Smell. I just suse my hands to put diaper cream on and cover it like a thick paste all over.

I always use a wipe but I know daycare doesn't use a wipe for just the pee diapers (since we use cloth and only get wipes back with the poopy dipes). I just use my finger for the cream and as far as sizing up I tend to just follow the weight guidelines and when they start to feel snug.

I only use wipes (water only) for poop. And I use my fingers to apply coconut oil after every poop + if he's red/rashy.
Go up a size in disposables when they feel snug/are leaky around the legs. We always have poop blowouts with disposables, so we can't use that as a fit gauge.

@Foodnerd81: the Vaseline/diaper rash is crucial till they're done with meconium poos. After that its as-needed.
