Just some random ideas:

New bottles (if you formula feed or pump) and nipples, pacifiers, blankets, bath items (shampoo, etc.), wipes (if you use 'sposie wipes), baby medicines or ointments, crib sheets, a new carseat if yours has expired.

Cloth dipes for burp cloths? Do you cloth wipe - disposable wipes could be useful. White onesies (s/s, l/s), books (board and paper), lotion/soap/diaper rash cream, sheets and flannel mat pads for under sheet/swing/pack and play etc?

How about things you need duplicates of - another car seat, high chair, baby carrier, more crib sheets? Are there any items you didn't have last time that wish you'd bought, like a swing, bouncer, jumperoo, bumbo?

Gift cards are a great idea. Also maybe you could register for some toys or items you need for your older children?

Definitely just gift cards!

Bibs, burb rags, sheets, changing cover, anythign that culd have been washed/worn/or stained from the last one!

A new, bigger diaper bag! Storage stuff for inside the diaper bags, like wetbags are always good.

We just did this for a friend of ours (team green with #2). We actually just did a shower after the baby was born. It worked out great because her second was a boy (first was a girl), so we were able to get her some cute blue stuff!
