Have they run any blood tests? My thought with increased urination (although I don’t know if it would link with painful urination) and joint pain and just general unwellness would be diabetes. I think reoccurring infections like UTIs can lead to a type of reactive arthritis that would cause joint pain but that is very uncommon in children (my elderly father had an issue with this years ago). I’m sorry to hear she’s been feeling unwell - I hope the specialist can pinpoint something.

@Iced Tea: My daughter one time had all the symptoms of a UTI and it ended up that she had strep throat! The only reason we figured it out is because her awesome doctor said strep had been going around and had urinary symptoms for some kids. It was so weird. She never had a sore throat but her symptoms cleared right up with treatment.

That was very unusual obviously. A less unusual reason could be dehydration. I have UTI-like symptoms when I’m dehydrated.

@Iced Tea: Sorry, going to rattle off a differential diagnosis in no particular order of suspicion: Diabetes, Wilm's tumor, UTI, pyelonephritis, nephrotic syndrome, Hemolytic uremic syndrome, viral exanthem.

There are a number of possibilities. In adults, they automatically assume, if not a complex UTI, it must be a STD; however, with young children, physicians need to explore outside of the box. It could be viral rather than bacterial , therefore, their tests would not pick up on a bacterial UTI. Perhaps, a severe allergy to soaps or detergents that only affects your daughter's "sensitive areas" (lower mucus membranes) but so much so it is a catalyst for other symptoms. They can even cause back pain.
More ideas: A recurrent kidney infection?
Maybe it is a fungal infection ... Bacterial tests would never pick up a fungal infection. Also, although fairly rare in children, it may be interstitial cystitis which mimics UTI. Another two possibilities are pyelonephritis & diabetes.

@dc yoga bee: Omg.. another thing I will now be paranoid about!! If the redness went away, that does *sound* like a YI. I hope it is, so she doesn't need any more abx. Fingers crossed for you and her!

@Iced Tea: I am sooooo sorry you are going through this. I have no advice but I just want to reach through the internet and give you a hug. How old is your daughter? My just-over-2-year-old currently has a UTI. Symptoms were throwing up and a rapidly rising, really high fever (got to 105.5!). The ER experience was traumatic (blood draws, catheter, intramuscual shots in oil, etc.). I still can't stop crying over it all and I am feeling so much pain for all the moms who have sick kids. I am praying your daughter recovers and has nothing too serious going on. Please keep us updated.
