Mayo and pretzels are my two most hated 'foods'.
I don't like to drink milk, but it doesn't bother me in other forms (I can use it in cereal and put it in smoothies).

Avocado
Almost all seafood save a few select types of fish, and even that is hard to down. Shellfish is out entirely.
I have a psychological allergy to them, lol. Even the thought of eating something like shrimp makes me want to gag.

Any not horrible for you pork (i.e. I like pepperoni, bacon, and sausage, but not ham or carnitas or white meat pork)
Any beef other than hamburgers or hotdogs
Any poultry with any sort of sweet glaze/sauce (fruit, bbq, teriyaki, etc)
Almost any cooked vegetable
Basically if you can't get your average 2 year old to eat it, you can't get me to eat it (and sometimes even then… lol)

I'm kind of proud that I'll eat (and enjoy) most anything except extremely exotic foods with bad reputations (Durian), where "rotting" is part of the preparation process for meat, or food made of unappetizing animals parts (like blood soup or headcheese). I wouldn't eat a lot of what's on Bizarre Foods, but I try to take the mindset "if it looks good, eat it" to heart.
I hate eating food off of bones, but I enjoy the taste, if that makes sense. I really like chicken wings, but I hate the act of eating them off the bone. I like to pick them apart and then eat the meat.

The only two foods that I really dislike are celery and pineapple. I will eat them sometimes, but it really depends on how they are prepared.

@temperancebrennan: I'm with you. I don't like to eat food on the bone. I'll cut or pull it off and eat it, but won't bite into that way. So I don't eat chicken wings, 'cause it's not worth the work for me
