My biggest fear about childbirth after my first BFP was ending up with an emergency C-section. Unfortunately, I did end up with a C-section! 
My biggest fear after my 2nd BFP was another complicated C-section with failed anesthesia. Fortunately, my surgery was pain free but I had a PP hemorrhage.
What was your biggest fear?

C-section, definitely. Thank god for British doctors, because an American one would have cut me open in an instant.

The pain of contractions, and if I would need a csection, was especially scared of the c section because I would have been knocked out completely and miss the whole thing. Luckily it wasn't needed.

First time - a cesarean (got one)
Second time - a cesarean (almost got one but got a vbac)
This time - transferring! I no longer worry about a cesarean, but do NOT want to go back to the hospital!

With #1, a csection...which I ended up having. But I'm completely ok with my csection, I truly believe it was the best.
With #2, I'm worried about complications during or after my csection due to my births being so close(15 months)

Tearing. I heard such horrible stories. Thankfully, I only had a slight tear that only hurt for a few days and required only a few stitches.

Tearing! Ughhhhh!!! The thought of my vagina ripping scared the crap out of me'
I was lucky and my dr did a ton of perinneal massage and I was so comfortable due to an awesome epi that I was abe to labor down a long time and DD was practically falling out of me. No tearing! 5 pushes... Just some skid marks and scrapes!!

I'm not even pregnant yet, but I'm always so afraid of tearing! Also I have a very low pain tolerance, but I'm petrified of needles, so I definitely am hoping to stay away from an epi. But who knows!

My biggest fear was that I'd end up with a scheduled C-section after paying for my doula. I really struggled with spending her fee on myself since I saw it as a luxury. I was afraid the money would be a waste since she wouldn't have been able to be in the room or anything. Turns out I was in labor for so long she barely got minimum wage. So I guess I got what I wanted.
I was so naive and strangely confident about all of it. I'll have so much more to worry about when/if I do it again.

Tearing. Luckily (or unluckily) LO came early and was tiny and I only got one little tear that healed pretty quickly.

Tearing. I did tear, and had an episiotomy, but it wasn't as bad as I had imagined. I even pooped that night!

not having enough time for the epidural! (I thought I'd have as fast a labor as my Mom did).
aaaand lo and behold - I almost didn't have time to get the epi - they gave it to me fully dilated

Tearing badly or needing an episiotomy. I really trust my midwives so I feel like if they say I really need a c-section, I do, so I'm not even worrying myself about that, but the tearing I'm nervous about. That and being incontinent afterwards--I've been lucky so far and only peed on myself a little once, when I sneezed on my way to the bathroom!

Btw... I'm more scared for #2's birth than I was for E's. His went so well that there's no way we could have 2 perfect ones! Plus we have to change hospitals. I'm basically terrified.

I'm not going to lie, I'm pretty freaked out about the whole process, the epidural, having an emergency c section, tearing, ya know the whole shebang basically.

@autumnlove: Me too. *sigh* I have really surprised myself though . . . I am so silly for LO (I didn't know if I had it in me!), she is my little buddy, and she is totally my heart. How I feel now (she is almost 11 months!) gives me hope.

I thought having a c-section (a) was the end of the world, and (b) would never happen to me. Turns out I ended up with a c-section and was wrong on both counts.
My c-section recovery was the easiest thing ever. No pain. Very minimal healing time.

That I won't actually go into labor on my own again and need to be induced...again. My body did nothing on it's own last time, but a still delivered a healthy baby vaginally in the end.

I wasn't afraid of the pain, I was afraid there would be something wrong with the baby that they weren't able to see during the sonograms (even though I had a healthy pregnancy.) That and what @rubies and @chopsuey119 said.

I was scared of an epidural, episiotomy, and a c-section. I got the first 2 anyways.
I'm not going to care about any of that next time around. I don't even care if I have to have a c section for a healthy baby to take home, all I am scared about next time around is losing my baby again during labor.

C-section, pooping, barfing and tearing. The first three didn't happen, but when I was in labor, I couldn't even think about tearing, I was so focused on getting LO out! The tears weren't too bad, I think I had 2 stitches.

I was scared of tearing, induction, and needing a section. Fortunately the last 2 didn't happen. I did tear pretty badly, but I didn't feel it when it happened, and the recovery actually wasn't too bad.

Episiotomy or csection. My little darling was posterior and required an episiotomy to birth vaginally. I guess it was the lesser of my two fears.
I thought for sure I'd feel it since I could feel contractions through the epidural but I didn't feel the cut at all.

I didn't really have any- I was super naive and thought it was going to be a piece of cake!!! Turns out, not so much!!!!!
