Both times I was in the hospital for like 4 days so while some may think it was over packing I feel it was just right.
Me:
-bath robe
-fuzzy socks
-2 to 3 sets of pj/yoga pants
-2 to 3 tanks
-1 custom hospital gown (given to me as a gift. Super nice for right after the birth but I wore regular clothes the rest of the time.)
-going home outfit
-toiletries (makeup, toothbrush, starightener)
-head phones
-Kindle for Netflix
-pillow
-charger
-the second time I also brought some of my own snacks
Baby:
-diapers
-2 to 3 newborn outfits (I suck at changing new born diapers, I get poop everywhere lol)
-2 zero to three month outfits
-blanket
-pacifiers
-going home outfit
-hat
-socks/mittens
-nipple cream.
I'm an over packer by nature but considering how long I was in the hospital it was nice to have all that stuff.

All I used from my (birth center) bag were my newborn coming home outfit, my toothbrush, a phone charger, and some snacks!

The thing I read on hospital lists that I was glad to have in my bag was lip balm! That's a must have for me. All the breathing in and out made my lips really dry and I was applying it every two seconds.

Hair brush
Nipple cream
Toothbrush and toothpaste
Baby:
Diapers
Newborn outfit and 0-3 month outfit

I feel I packed pretty sparsely, but I live 15 minutes from the hospital and I ended up getting my sister to bring me nail clippers for my baby's super long nails, prunes, tweezers for me, and some salad. I also wore the hospital gowns the two days and nights I was in there. I had an ubexpected c section and when I got dressed for going home I discovered that clothes were pretty uncomfortable.
Must haves for next time: maxi dress, lip balm, straw cup.

@Mrs. Goose: Yes, lip balm. I remember how happy I was to have this with me! I think I brought several tubes just in case.
And yes with my first I had an unexpected c section and even my maternity jeans were very uncomfortable to wear home so with my second pregnancy I packed maternity leggings and a loose tunic and was much happier.

I had a quick stay (1 night), but I found that wearing my own pj's was a huge comfort.
my must haves:

4) bath products and minimal make up to feel refreshed
I didnt pack much for the baby and found that the baby didn't really need much - I brought one newborn outfit and one 0-3 just in case.

I totally over packed for my first too (but don't regret it; helped me feel prepared).
@mamabolt: Totally agree that these are really the MUST have's!
Our hospital literally had EVERYTHING.

Here are the things I actually used
For Me:
- phone and wallet and birth plan
- large heating pad
- hair tie
- gatorade
- something to play music
- Popsicles
- snacks
- dress with easy access for nursing
- camera
- toiletries (including chapstick!)
- fuzzy slippers
- long charging cord/extension cord
- pillow
- champagne
- breastpump (to practice with a LC there at the hospital)
- yoga pants, Tshirt, undies, nursing bra for going home
For baby
- 1 newborn footie pajamas
- 1 0-3m footie pajamas
- carseat

@mamabolt: Ah, pillows are the other thing I forgot that I found necessary. I had 4 hospital pillows in my bed so I could get comfy and I had brought my body pillow from home. I would definitely bring my body pillow next time and probably a nursing pillow!

So I totally procrastinated in packing a bag for my second until I was in active labor and threw something in while having heavy contractions. Who knew this one would come early?
All I really wanted was my burts bees Chapstick (I'm obsessed) and after that I just needed yoga pants and nursing tank top and a cardigan on the way home. I forgot most of my toiletries so used what the hospital gave me and what I had on me in my purse.

Me:
I wore cheap flip flops into the hospital because I knew I would need them for the shower and didn't feel like packing extra shoes. I also suggest a dark colored towel from home - the hospital towels are so tiny and that first shower is magical.
Chapstick
Hair ties/clips/bands/whatever you prefer
Comfy pants, nursing tank, zip up hoodie to cover up
PJs or nightgown and robe (I brought a zip-up duster robe, so it was like a 2 for 1)
Long iphone charger
Eye mask and ear plugs
White/clear Gatorade
Pumping bra
Pillows, blankets, towels for DH
A small makeup kit and brush
Hand cream
Deodorant - so much sweating
2-3 outfits for baby in NB and 0-3

I'm adding a tennis ball. At my hospital'hospital's birth class the nurse demonstrated counter pressure using a tennis ball and it was heaven.

My own pillow.
Deodorant, hair elastics, body wash, toothbrush & toothpaste.
Baby:
Coming home outfit in both NB & 0-3 (pretty sure it was just a footed sleeper). They stayed swaddled in a diaper & blankets the entire time.
Blanket
If they were winter babies I would have also packed a hat.
