I'm on Android and using FeedBaby - it tracks all that and syncs with dad.

I liked the BabyConnect app. I still use it to track my kids' height/weight/head size at their physicals.

I use baby tracker. I used it for my first too. Can sync between multiple Apple phones and iPad. Stores e data and allows multiple children so I can refer back to my daughter's info which is kind of cool. I only used it for about two months, but quite like it.
It allows for all the things you want plus milestones and growth, etc. And you can put notes with each entry for diaper, feeding, sleep, etc.

@bloved: @StrawberryBee: I'll "third" Baby Connect. I used it daily with my twins and it was AWESOME. You can get super detailed if you want - but it has all the tracking you were describing. We did diapers, nursing/bottles, medicine, and measurements from the doctor (head/height/weight) pretty consistently for the first year. We have backed off now to just measurements from the doctor - and this past summer started logging naps.
This all coming from someone who literally NEVER pays for apps. This one was worth it!

Baby Connect! It was invaluable when I went back to work, so that my mother, husband, and I could know LO's eating and sleeping schedule for the day without having to exchange a million notes. We still use it to loosely track LO's sleep at 21 months, and, as others have noted, to keep track of stats and vaccines at check-ups.
I cannot believe that I kept track of everything with paper and pencil for the first three months!
