Baby gates. I never had them with #1 because I was up her ass all day but with 2 little ones going in opposite directions I'm thinking it will be necessary.

With a second it will be important to have a place to set down the baby in each area of your house - so bouncy seats or whatever if you don't have enough "baby holders"

Also a baby carrier will be more important so make sure you have one that works even for a newborn.

Additional cloth diapers if you use them and the older is not potty trained. Newborn diapers and wipes, too.

Double stroller

Nursing bra and nursing/pumping supplies. Don't forget to get a new pump from insurance (assuming the option will still be available). Or formula and bottles if going that route.

New diaper bag? I needed this with my third and could have used it with my second. And all the baby supplies like diaper cream, hand sanitizer, baby first aid kit, etc.

New bibs
I should have gotten new nipples for our bottles but we don't use them too much.
New pail liner for CD
Pumping bra (I love this! Didn't have one for DS 1).

@littlejoy: We've been reusing bottles and nipples. The silicone on the nipples is looking a little old. The bottles are fine. If we did bottles more than a few times a month I would get new parts for the bottles.

@littlejoy: Yeah.. Last time I guess I was apprehensive about how long I would make it breastfeeding/pumping so I was afraid to buy more stuff... This time I'd like a few more bras/tanks so I can actually wash these nasty things more frequently lol!

Uh are you sure? I'd love to host a shower for you. Or a brunch at that's place across the street.

Milkies milk saver (since I was really leaky with DD1)
a double stroller with a new infant car seat
Rock n Play to replace our large swing
Swaddle me by your side cosleeper
Baby Bjorn (tried wraps last time and didn't like them)
New Petunia Picklebottom diaper bag since my old one was gross and the seam ripped
New swaddle blankets (just because they were cute)

Luckily my girls are almost exactly 3 years apart so I needed pretty much zero clothing. I might have to invest in some plain onesies to go under her sweaters but I probably won't have to buy anything until she hits 9 month size!

A second camera for DS2.

Baby bum brush + tubs of Triple Paste and A&D

A second Nose Frida

Gripe water.

A new set of pump parts.

Simple Wishes tie back bra (the velcro one wore out on me!)

Rumina pump and nurse tanks (I don't like their bras)

A new humidifier.

PBK hooded beach wraps as bath towels.

We just bought a Joovy Caboose on sale.

DS2 sleeps in the PNP with a Dream on Me PNP mattress, so we got a few extra sheets for the mattress. (DS1 just transitioned out of his crib).

Land of Nod hanging canvas organizers + drawers for clothes.

Season appropriate sleepers (fleece sack and some fleece sleepers for DS2).

Zutano booties.

Yeti-type tumbler for my constant gasping thirst.

A Swell bottle for hot water to mix warm formula on the go (if necessary).

New pacifiers and bottles.

Some new stuffed animals and loveys (not everything can be a hand me down, yanno?) The SkipHot blanket rattle has been a favorite of the new baby.

A new splat mat.

Feeding bibs.
