All the family names on my dad's side are super old school! Basically, a bunch of French and German names that nobody uses anymore. My brother has one of our family names as his middle name, and it's one of those names where you use your initial for your entire life.
My mom's family names are great, but it's hard to pull off a Japanese name outside of Japan these days!

If we had a girl we would've used Natalie and my middle name. I have my great grandmothers maiden name for a middle name and share it with just about everyone on my moms side, male & female.

Georgia - my grandma's given name although she went by middle. I love it, but DH vetoed immediately. He thought it sounded "too southern," ha!
Wilfred - my grandpa's name. We're not cool/hipster enough to pull it off. We considered it for a second though!
Heath - DH and his dad's middle name and DH's grandma's maiden name. We thought about it, but it just didn't feel right to me.

I wanted to honour my gran (Audrey) with DD's middle name but DH wouldn't entertain it at all. We didn't use any family names but all of my sisters have honoured my parents with theirs and two used our Nan's middle name (Mae).

My great grandma's name was Marcella. I loved her so much, and it would mean a lot to my mom. I dont hate the name; i just don't know if I love it.

@LovelyPlum: That's my mom's name! She was named after my grandpa's favorite cow from childhood (what happens when you're the sixth girl!). She goes by Marcie.

SIL took the family names from DH's side that we liked, and I don't really fancy the women's names (Dorothy, Janet etc.)
So we're on our own. My side has cultural names with English equivalents that aren't common, so nothing there.

I am the fifth in my maternal line with my name, and I broke tradition by not giving it to my daughter.

My great grandmother's name was Delbert. She went by Berdie. VETO!
DH's grandmother was Jadwiga. Nickname Hettie. Still, VETO.
We went with a different family name for DD's middle name, and have a non-family first name we adore.

Harold. There was a generations long tradition of naming the first son Harold. My parent got around it by getting a male kitten while pregnant with me (dad was convinced I was a boy) and naming him Harold.

I broke the tradition of the eldest females middle name being Janet. I was the ... 5th person with it I think. I'm not very fond of it, plus o have no idea who the original Janet was. So for my oldest daughter we used my nans name and my youngest daughter we used my moms name ( for middle names.) It works out though, as my sister said she would use Janet as it works for our family, plus is her mil's name.

My beloved grandmother was Linda. I don't mind the name, but it's not my style nor my favorite, and I haven't found a good way to use it as a middle.

DD is named for each of our favourite Grandmothers. Helen for his, and Katherine for mine. Katherine is my Grammies middle name, and is a family name. I have a ton of female relatives with this as a middle name.
Next time names from our family we may consider:
Arthur (Hubby has his heart set on it......)
Henry
John (My hubby, his father and Grandfather all carry John in their name. Hubby wants to give our future son it as well)

Sadie

Oh gosh yes, we have some terrible names in our families! Calman, Perry, Frederick, Betty, and Renee for example are ones I just wouldn't use

Marceline (because it is already my first name) it was my great-grandmothers name and it is also my Mom's and cousin's middle name.
And Richard. Because well it has a very unfortunate nickname.

I wanted to use a version of Frank or Frances as it would knock out two birds with one stone. DH was on board with Francesca for hot second. He didn't have any family names he wanted to use.
