I usually take them out. Also wine, or something for their kitchen, depending on who they are. I've also given some necklaces, but you have to know their style. Brunch?

We just celebrated a birthday "trifecta" and I got each of the girls a Naked palette, they were all excited. I've also done clothing and jewelry (stamped pieces with dates/names/ect.), gift card for pedicure/manicure, and massages.

We usually just go to dinner and maybe give them a gift card to a place they like to shop or if they mentioned wanting something, some of us will go in on it together.

I can't remember the last time we gave each other gifts for our birthdays - it has been a few years. We treat each other to girls night out or brunch. I have one friend who's gotten me a lot of home decorating items - candles, table runner, things to put on my fireplace mantle that I have enjoyed.

I only really exchange gifts with my bestie and we both give the same gift - large bottle of wine and babysitting so we can go out with our husbands on our birthdays. Usually we go out to eat/celebrate with each other too on another night

I'm totally a gift-card type person. I don't do birthday gifts for many of my friends (just one or two), and I've gotten an Etsy gift card for one of them, and an LCBO card for another. That being said, going out is another good option, or treating to a mani/pedi.

We try to do experiences. One year we did a hot air balloon trip, another year we went salmon fishing. Mani/pedis or brunch too.
For friends who live further away or who can't get together I like fancy lip balm, hand lotion and fancy chocolate. Or crazy socks, good coffee and a snacky treat.

Gift cards/certificates to their favorite restaurant, store, nail salon, or spa for a massage. Or Sometimes, I'll take them to get a mani/pedi.

I don't really do birthday gifts, just dinners if it comes up. I am not a big gift person. My husband and I don't exchange gifts either.

Since I had LO I've hosted two parties for people in lieu of a gift just because it was easier to have everything at home with LO's schedule.
We often gift books, speciality art supplies, home decor (usually art prints) or wine.
Gift cards are often easiest to gift and not exactly impersonal in my opinion.

I'm sorry, I didnt read everyone elses responses so their may be repeats but here are what I have done recently:
-Fall scented candle, w/ startbucks card and mini loaf pan filled with pumpkin bread
-Bottle of wine & box of chocolates
-Gift subscription to stitch fix (could do the beauty boxes, nature box, fitness boxes)
-Flowers, great for long distance
-Lush gift package (handful of nice things from Lush)
-Cute scarf w/ coordinating earrings
DH usually gives either a nice bottle of scotch, giftcard to a local craft beer store or cigars to his buddies.

Drinks if they're local, fancy chocolate or booze or a spa gift card if they're not.
