This totally happened during our newborn photo shoot...but it was when I'd stopped to nurse her. Fortunately, I'd taken my dress off to nurse, since it wasn't very accessible. I had to summon DH to run to our room and fetch me a clean bra and undies before I could get dressed again and join the photographer, who was shooting in our bedroom.
