How many of you applied for short term disability during your pregnancy or maternity leave?
I am planning to do this since my company wont pay me while I'm away. Did you call in to get them to fill out their section of the form after you stopped working or did you have them fill everything out in advance and so you had all the paper work ready to mail in once your last day rolled around?
Also when did you have your doctor fill out their section. I'm not sure if I should bring it up now? Or wait til I actually have the baby

HR is taking care of most of it for me. You want your doc to fill it out now. I filled out my portion then my doctor filled out their portion. Now HR has the form and she will be sending it in when I let her know I go into labor.
Do it now so it's one less thing to worry about post baby.
In NY state at least that first week of my 6 weeks is a UNPAID waiting period.
It took my co-worker about 3-4 weeks to receive her first check.

It depends on your state; in CA, you have a one week waiting period that goes off of the date of the event (i.e. birth) so you can't really fill it out in advance (since you need the birth date). Also, in CA, there are different payment periods for vaginal birth than for a c-section, so you wouldn't want to fill it out ahead of time, in case you needed an emergency c-section and the information changes.
I took the paperwork with me to the hospital, and filled out my part in the hospital. Then, on the way home from the hospital, I dropped it by my doctor's office. She completed it and mailed it in for me the next day.

For me, I had to fill out all the paper work ahead of leaving. I would check in with your HR to be sure.

I filled out all of my short term disability and FMLA paperwork about a month before the baby came. My HR rep gave me the paperwork, and after turning it back in after the doctor filled our and signed his part, all I had to do was tell her when the baby was born and she would finish off the paper work.
The STD rep called me to confirm whether the birth was vaginal or c-section (2 weeks more benefits for a section) and asked a couple more questions, then the money kicked in a few weeks after birth. The funny thing was that when the rep called, I was in a sleep deprived state almost a week after birth, and didn't understand who she was, why she was calling, and why the heck she wanted to know how the baby was removed from my body. I was like "nosy chick" until I realized who she worked for.
I definitely recommend doing it if you have it available- the money was so helpful, and I had totally forgotten we had it as an option.

My company HR are a bit sloppy so i wanted to mail it in myself to be sure its sent in.
