Bring a thermometer and Tylenol with your pediatrician's suggested dosage chart.

We traveled to Eastern Europe and some of the cars there don't have the same seatbelt locking mechanism as most American cars. We needed an extra seat belt clip to accommodate this.

Take twice as many diapers as you think you need in your carry-on. We took LO1 to Colombia when she was 5 months and our first flight was cancelled, so we missed the connecting flight. We got back to our home airport about 6 hrs after we expected to and I used our absolute last diaper when we got off the plane.

Probably not what you want to hear but most cars/taxis I took (lived in Chile/Bolivia/Ecuador for 4 years) had no seat belts. I've also never seen anyone use a car seat. I also saw very few strollers--most everyone seemed to baby wear. On the plus, everywhere is very accommodating to children.

You'll be ok The anticipation is always more stressful. For me at least

1) book a seat for your LO if you can. And bring the carseat on the plane.
2) we find that hotel rooms that are suites are better for naps, night time, etc. so we can still talk and watch TV while LO is in the bedroom.
3) for the flight, take 2x as many diapers as you think. I've also heard 1 diaper per hour of travel. We are currently abroad and took a 10 hour flight here. I brought 12 diapers in my carry on.
4) snacks, pouches, etc out the wazoo for flight and hotel. And embrace that LOs diet will be less than ideal. We have been here a week and he has been living on pouches and crackers. And some fresh fruit here and there. Between not always sitting down to eat and new foods and schedules being out of whack, it is what it is.
5) regardless of your view on screen time at home, on the plane it is your friend. No kid under two has died from screen time. You'll be able to shove a snack in your pie hole, watch an episode of Real Housewives or even shut your eyes for 30 mins.
6) As tempted as you were to carry as much as you can on pre-kids, ignore it and check whatever you can. Take just the car seat and one duffel bag/carry on. We actually do one duffel bag for me/LO and my husband takes a backpack with electronics (all of ours). That's it. The one carry on has basically all of LOs stuff (snacks, diaper stuff, toys, books, change of clothes, etc) and a couple of my things (change of clothes and maybe my makeup bag because I don't want to lose that!) To just take one thing on makes a huge difference. Bonus if it can go cross body so you are truly hands free.
7) Get a diaper clutch or use a men's toiletry bag or even a ziploc for diapers, wipes, ointment and a receiving blanket. It's much easier to have one "go bag" to take to the bathroom with a toddler than to have to rummage and juggle multiple items or take the huge duffel bag. And don't skip the reviving blanket. The airplane and airport bathroom changing tables are N-A-S-T-Y.
7.5) I've also found that for the diaper changes it works best if DH and I both go back. I go in and change DS, then when he's all ready I hand him to DH and he takes him back to the seat. I can then clean up the diaper stuff and maybe even go pee myself without worrying about my toddler licking every surface in the process.
8) get one of those roller things for your car seat (if you're able to get a seat on the plane and bring LOs car seat). It is easier than lugging it and you can then check the stroller and wheel LO in the car seat as a makeshift stroller.
9) 1-2 changes of clothes for you and for LO. Puke, blowouts, messy meals, etc do happen. And sometimes you just feel crusty and want to change. You will regret not having a change of clothes if any of the above happens.
10) dress LO in layers. T-shirt with a hoodie, for example. First on the plane it's boiling and then once AC is on its freezing.
11) toys - bring 3 or so faves (small non-noise) and 3 new ones that you unveil on the plane. Same with books if you have the room in your bag.
12) foogo thermos or funtainer are awesome. Keep milk cold for like 12 hours especially if you toss in an ice cub on the plane. You can just keep refilling it with airport, airplane, or shelf-stable milk.
13) shelf stable milk is awesome. I brought 3 8 oz cartons on the plane on the way here and he really didn't use any of it. So I'm only taking 1 on the way home tomorrow. Depends how much of a milk monster your LO is. But between milk at the airport and on the plane we didn't need them. But I don't want to be caught with none!
14) definitely take any medicines for LO on the plane with you. And don't forget the syringes! Motrin/Tylenol, Orajel, Benadryl, thermometer, etc. Def don't want to be stuck on a plane without those.
15) pacifiers in every pouch, pocket and bag. We limit them to nap and bedtime in the crib at home. But on a flight or airport they are unlimited and it's good to have one in easy reach at all times.
16) pack a straw cup (empty) for water in the carry on but don't fill it till after you're through security.
17) in each hotel room, unless you're there for like a night only, unpack and organize what you can. I like to know where LOs stuff is instead of rummaging through bags. I pick a drawer (nightstand for example) where I keep diapers and wipes and a blanket. And that area of the bed becomes my changing area. I set up my travel bottle brush and drying rack in one corner of the bathroom counter (oxo tot makes a great one). Etc. That way too if DH is taking care of something for DS I can tell him exactly where to find what he needs instead of saying "it's somewhere in the big suitcase. No the other suitcase. In the side pocket. The other side pocket. Here, just let me do it!"

I know this is an extensive list but we literally are in the midst of international travel as I type this so its all fresh in my brain. LO and DH are both asleep so I've got free time I'll add anything else I can think of.

We did a 14 hour flight (direct) to Korea when LO was 10 months old.

Try to get a bulkhead seat. Our plane had a bassinet for bulkhead seats and she barely fit but was a good place to lay her down when she fell asleep. Also, you have additional room on the floor. We put a blanket down and she played with an empty Starbucks cup for a good hour on the floor.

I was BF at the time so I invested in a travel Boppy. That helped during the flight and she fell asleep on me on top of the boppy several times. My arms were glad that I had it. Even now with my LO2, I use it downstairs.

Baby wearing is a huge plus. We used Ergo and during the flight, my husband walked LO back and forth down the aisle and it helped her fall asleep.

We normally use the britax stroller but we bought a cheap umbrella stroller for the trip. The sidewalk, public transportation, stores, etc was much easier with a smaller stroller.

We bought a used car seat while there. My father in law drove us around in his car so the car seat was necessity. If we had brought our rear facing car seat, it wouldn't have worked.

We traveled a lot while in Korea so we also brought along the portable crib. She slept in it like a champ.

@Dahlia: We traveled last November with our 10 month-11 month to Ecuador! It was a great trip. We were staying with family and sometimes when they planned our car trips there weren't enough seats to use the car seat. Just something to think about how you would react.
