Empty drinks is my biggest restaurant pet peeve. The servers who ask if I'd like a refill when I still have half a glass left always get much better tips from me.

Waiting for the check and waiting for drink refills. Especially for soda refills. If I'm paying $1.95 for a soda, you better be on top of getting it filled.

@JennyD: interesting - I don't think I've ever been to a restaurant that *did* bring any kind of snack to my LO upon us sitting down! I keep a ziplock of goldfish/cheerios/snacks in my bag in case she's hungry, but that's definitely not something I ever thought to expect a restaurant to do.

@yoursilverlining: Not upon arriving, but once we order. We usually order something like grapes or goldfish, and a lot of the time, servers will bring that out right away, which is nice. I also hate it when the prepare the kids meal and let it sit until our food is ready. Then the plate is smokin' hot.
If we're going out for dinner, it's usually because we're incredibly disorganized and therefore I don't have snacks in my purse

Loud people, especially if they're on their phones. There was a table of really loud women beside us last time we ate out, and we had to work really hard to block them out.

I'm not a fan of over-attentive servers. I appreciate a drink refill and a check-in or two, but please don't hover. There's a balance.

Servers that interrupt me when I'm in the middle of a sentence, just to ask how everything is. Or when they do that right after I've taken a bite. I've noticed a HUGE difference at the super-nice restaurants where excellent service is a hallmark and the servers are trained not to interrupt. It's a little thing that makes a meal so much more pleasant.
On the other side of the coin: eating with people who are rude or demanding or grouchy toward the servers. I can't stand that.
[ETA] Actually, probably the thing that annoys me most is when restaurants are too loud for me to hear my companions' conversation.
