Our first apt. was an orientation. So they went over tons of stuff, and we just asked questions that we had as we went along. They talked about foods we can't eat, the hospital we were delivering at, when to do things like sign up for childbirth classes, when to register for your baby's birth, our medical history, etc.
I wouldn't worry if you don't have a specific question when you go. Chances are you'll come up with one, or if you have questions later you can always call up and ask.

Ours was similar to ArtBee - she gave us a big packet of info from the hospital we will deliver at, gave us other pamphlets, told me to take either a prenatal or folic acid pills, and gave a sort of overview of what was happening at that stage and what sorts of visits and tests would come next.
I asked about fish/seafood because we eat a lot of fish and seafood, and she gave me a brochure on good fish/bad fish made by our state (and the FDA?), and told me her opinion.

I knew early on that I wanted to have a natural childbirth, so some of the questions I asked during my 1st appointment were to find out to what extent my doctor would support that decision. I knew I would want to switch to a midwife practice if I wasn't comfortable with my doctor's opinions of natural birth. It turns out I really like my OB, so I ended up sticking with her.

I have my first appointment on Monday and I have started a list of questions to ask. I've done a lot of research online about stuff but I just feel better getting one answer from the doctor vs. multiple answers online! I'm asking about the changing the kitty litter and about my cat sleeping with me. I want to ask about exercise and what all I should be doing to stay active. Also, I have a couple questions about different makeups and chemicals. I know once I'm there I will be asking a ton!

Our appointment was just like @artbee's. They answered a lot of questions I didn't know I had. I really didn't ask much because I feel like I don't know very much and they'll tell me what I need to know. I still don't have a lot of questions. I just think some people are more prone to finding and asking than I am. I'd say if you don't have any questions you know you want to ask, go with it. Go through your appointment and see if you come up with some when you're there. You can always call in with more questions later.
The one big thing they'll ask you is if you want to do first trimester screening. I would do some research on that first and if you have questions on that, you can ask them.

@skibobrown: I know this is an old thread ... Gearing up for my first appointment now. Did your OB end up following through on their support of your natural birth? I'm in the same situation & curious!
