When you say preschool birthdays, are you just talking about the age (close friends), or kids you don't know very well (ie preschool classmates)?
It totally depends on budget - for something very small, the Melissa & Doug reusable sticker books, stamp/marker sets, etc. are always a big hit. For awhile I was getting all the girls tutus/tutu dresses and some sort of accessory (big headband, wand, necklace, etc.). Also, if you want gender neutral things to have on hand, games work for kids who are a bit older (some 3 year olds and most 4 year olds) - The Sneaky Snacky Squirrel, Chutes & Ladders, Memory (there are various themed versions like Marvel Hero), CandyLand.....
I really do try to cater to the individual child's interests if I know them well, and I try to buy something their mom might not get them (but won't be too annoyed at me for buying, if that makes sense). I don't buy stuffed animals.
I've sometimes done a collection of craft supplies (cardstock, glitter glue, dot paints, pipe cleaners, pom poms, feathers, google eyes, etc.) - it's quick and easy to order everything on Amazon and toss it in a gift bag!

I like to do a theme...bath toys with fingerpaint soap, color tablets, or an art theme, or a skip hop backpack filled with a couple items, a book theme, etc. Just depends on child and how much I want to spend.

My LO just turned 4 so we have been going to a lot of her classmates parties this fall/winter. My go to gift has been the Hoot Owl Hoot game, unless LO has a specific idea (i.e. Laura loves my little pony, then we will find something with that). The hoot owl game is great because it is fun, cooperative, and not something most people have. LO loves it and I don't mind playing it over and over with her. They have it at target now too instead of just Amazon!

Almost always Melissa and Doug reusable sticker books. They are a huge hit with my kiddos and don't take up any space! Either that or bath crayons/paints.

I do puzzles or games. I buy ahead in bulk and make sure that the gift is appropriate for boys or girls.
This year, it was Blue Orange's Spot it Alphabet game and either a wooden cheese puzzle or story cards.
I've also given Ravensberger's puzzle ball.

For birthdays, I kinda assume everyone is in that same boat that they dont need more toys.
So I try to gift crafts, a useful toy (bath toys, sleeping bag, beach towel), or books.

@looch: Ooh, I thought of you, a game that we just tried from ChickFila called "Sort It". It gives you a list of four things and you sort based on the question (Sort these sports balls largest to smallest, sort these animals fastest to slowest).

@looch: So, we just have the mini version that came in a ChickFila meal a long time ago. But I'm thinking its this game? It says ages 8+ here but our version definitely had cards that she could do with help. We had a lot of fun. You get a Pink, Green, Yellow and Orange marker and put your votes in. Then she was able to match up the colors to see if she got it right.

I always stock up when I see something reasonably priced that'd be a good gift. So in our gift closet I have a few preschool-age games, a few craft kits and a few bath toys. I let LO pick what she wants to give her friend from there.
