Karla is my favorite if you want a feminine version. I agree with other posters, you could easily do Karl for a middle name for a girl.

My vote is for Karla - In my mind, it is the closest sound to Karl. With Karly/Karlie (I don't have a preference between the two spellings) the sound deviates from the original "Karl" sound more with the ee sound than with the a sound.
I wouldn't worry too much about the first and middle name flowing together. Choose a first name you really love and use this middle name that is so meaningful. I would hate for you to compromise on one because of the other.
PS. I know you said it wouldn't work because of your family, but I LOVE the idea of using just Karl. I love really unique, meaningful middle names.

My friend's daughter has a middle name of Karline (or maybe its spelled Carline, I don't remember) and its so pretty! I guess it depends on what the first name is but I think it is a good option!

Have you considered Karlene/Karleen? Flows well as a middle, easy to pronounce. I think Karline would get butchered a lot, but maybe not a huge deal for a middle name.
