I think it depends on the group you have. Our first shower was really casual - just DH's family. We all just sat around and talked while eating dessert. Opening the presents also took a while - to take more time have the mom-to-be pass around the presents as she opens them. (My MIL made me stop doing this after a while, because it was taking too long!)
I think the key is just to make people comfortable. Having some food and drink that people can graze on throughout the party, and encouraging people to talk to each other by introducing people and starting conversations will help.

I had a shower and have thrown showers with no games. People were happy just to eat, mingle, and watch presents being opened. That being said.... these were very big showers so presents took a very long time.

I think nothing is wrong with having no games. By the time people eat, mingle and chat, and then open presents that can take 2 hours. The only other thing I have done at a shower is we each put together a scrapbook page for the baby book. Since the Mom was into that. Can decorate onesies, too.

I threw a shower for my sister last month and had the guests decorate onesies using iron-on appliqués and fabric markers. The guests loved it.

I had a shower where we decorated onesies and I also specifically asked for no games. The onesie decoration was relally fun and entertained people for a long time. It was also a coed shower so they had football on in the background and people just mingled and ate. We didn't even open presents.

I like the idea of putting together a scrapbook for the baby. I bought my friend a wish book where everyone could add a piece of advice for mama or wish for the baby.

The one issue is that unless is a specific group shower (like, coworkers, or church friends, or just family etc) there always ends up being those few people who know nobody and just sit there awkwardly while everyone else mingles. I do think it's a good idea to have something to do.
It was a HUGE hit when I hosted and we decorated onesies... and not with fabric glue and markers lol we made little appliqués and each one was SO cute. It can be an expensive activity though because onesies aren't cheap. I bought mine at carters with a 20% off coupon but it was still like $60 for enough onesies for about 20 people.

@Coco Bee: YES good point. I attended a baby shower recently where the only two people I knew were the mom-to-be and the hostess who were both busy most of the time! I'm so glad there were activities to do.

I think having a lowkey activity like scrapbooking or onesie decorating is a good idea. My shower hostesses also had a photo booth at my shower that was a ton of fun. We had little baby related props and everyone loved taking pictures.

Onesie decorating was actually pretty inexpensive. I found packages of 3 onesies at Winners here in Canada (so maybe you can find them at TJ Maxx or Marshalls?) for $3.99 each. I think I bought 8 of them? Then I picked up the heat bond and fabric markers from Walmart - $10 total? Lastly, for the fabric I just ordered fabric samples from http://www.tonicliving.com for $1 each and used a 10% off coupon code that I found by googling. The samples were the perfect size and I made sure to order the most expensive fabrics! Haha.

Oh, and another idea I had but didn't execute was making a book of ABCs where guests would decorate a page corresponding to a specific letter. Much like the scrapbooking idea others have mentioned.

Oh thought of another activity - saw this on pinterest and loved it.
Buy a wooden and cardboard letter - each letter of the alphabet and each guest decorates them for the nursery wall
