I've always hated wearing shoes in the house but don't mind if others feel like wearing them in my house.

Never, but in Asian culture... it's the norm not to wear shoes in the house. I always ask if I should keep shoes on/off whenever I go to someone's house I don't know too well.

Never. We live in Asia so it's mostly customary to take your shoes off. Although, it doesn't seem to be as important here in china, as it was in Japan. When we were living in tokyo, even maitenance or delivery people, always took their shoes off without even being asked. The movers even took their shoes off. Here in Beijing, maitenance or other people wear booties over their shoes. But it's pointless because they have them on already walking through the hallways.

No! We have slippers or just wear socks. My house is far from spotless (I'll be honest, it's a mess) but I don't like "outside dirt" in. And yes, I know that makes no sense at all

No shoes in the house, even before we had a baby. Too germy. Although we do have 3 dogs who track dirt in from the back yard, somehow city/pavement dirt seems grosser.
