I only do about 60% of the time. And even when I do, it's kind of a cursory wipe.

I only wiped (and I have a girl) after a really wet diaper and following nap/bedtime. My sister has 2 girls and I guess I followed her lead and only wiped after #2 or super wet diaper.
My LO only ever got diaper rashes when she was teething (due to really lose stools) so it never seemed to be a problem.

I spray and wipe at every diaper change.
