Congrats! The biggest frustration point for me with my twin pregnancy was that basically I was considered high risk just because I was having twins. That required me to have an incredible number of appointments (ultrasounds, non stress tests, etc). I would want to really understand their layout of appointments that you can expect depending on what type of twins you are having. It was hard for me to understand what was normal for a twin pregnancy and what should prompt me to ask more questions.

The hospital I had my first at was the preferred of my practice for twin delivery since it has a level 3 NICU which I wanted access to even though didn't end up needing.

My OB also always delivers twins in an operating room and strongly encouraged an epidural in case I delivered one and they needed to manipulate the other baby into position or needed a c-section last minute. There were so many people present - normal arsenal of docs, nurses but then a NICU team for each baby.

And then I didn't really know my hospital's rules like they didn't want my twins sharing a bassinet at the hospital if we weren't awake. Totally legit but I'd read all of those stories about how tiny twins do better if they get to snuggle each other.

Basically my whole book above is that I didn't ask anything and felt unprepared every step along the way.

Congratulations on your twin pregnancy, @ElbieKay: ! My IVF di-di twins are 3.5 now, so my pregnancy is pretty far from my mind, by here are the things I would ask/look for.

- how many weeks will they let you go before delivery
- if baby A is head down, can you deliver vaginally even if baby B is not head down? (Ie, how comfortable is the OB with breech extractions of baby B?)
- will you be seen by MFM throughout your pregnancy, and will MFM do the high level scans?
- how often will you have appointments with the OB and MFM? Will you have a heartbeat check by ultrasound at each appt (my dr’s office does this for twins because Doppler checks of twin heartbeats can be tricky/inaccurate)?
- does the OB have privileges at a hospital with a high level NICU (my twins did not need nicu time, but it can be common with twin pregnancies)
- will you deliver in the OR at the hospital and what restrictions will this place on support people present at the birth (in my OR I could only have one person - DH - which wasn’t a concern for me, but I know it might have been an issue for others who wanted a doula with them, for instance)
- does your hospital have enough wireless monitors that you can walk around during labor and monitor everyone’s heartbeats? (My hospital had wireless monitors to use during induction, but not enough for me to have one for each baby, so I had to be tethered to the machines)
- will you be allowed to go without an epidural if you want? (My OB would have let me, but recommended against it because I would have to be knocked out with general anesthesia in the event of needing an emergency C. I ended up having an emergency c section for my son - baby b - so I’m glad I had the epidural placed and didn’t need to be put under general anesthesia!)
- do moms of multiples get a private room?
